Please notify Shelby Operating in writing if you have changes to any of the following:
•Change of Address
•Tax ID Number/Social Security number changes
•Name changes
•Transfers of ownership
If you have a change of address, please contact our Division Order department. We can provide you with a Change of Address form for you to fill out and mail, email or fax back to us. You may also download the Change of Address form by clicking here. We do require account changes to be confirmed in writing.
If you have purchased, transferred or inherited an interest in one of the properties we operate, please call and speak with our Division Order department to find out what information we need to make the necessary updates in our systems. We can provide you with the required forms to fill out and advise you as to what paperwork is needed to ensure your account is up to date.
